A cast iron light column sprang out of the ground and struck 57-year-old man Michael Hogan on the neck in Kilbride Plaza, Offaly, on 25 January 2019, leaving him paralysed from the chest down. Council worker Colm Scanlon had discovered the loose post two days earlier, and an HSA inspector found the removal methodology fundamentally flawed with no safety system, work plan, or risk assessment in place. Offaly County Council was charged with five counts of breaches under the health and safety regulations, pleaded guilty to two counts, and a nolle prosequi was entered on the remaining three. Judge Francis Comerford adjourned sentencing to 21 December 2021, with the offence carrying a maximum of two years' imprisonment or a €3m fine.
