Shane O'Farrell, a 23-year-old Irish law graduate, was killed while cycling near his home in Carrickmacross, County Monaghan, on 2 August 2011. The driver, Zigimantas Gridziuska, a Lithuanian man with 42 previous convictions, was out on bail at the time despite a January 2011 Monaghan Circuit Court ruling that he should have been in jail. Gridziuska left the scene but later turned himself in, and in 2013 pleaded guilty at Dublin Circuit Criminal Court to failing to stop at the scene, receiving a suspended eight-month sentence on condition he leave Ireland. In March 2025, O'Farrell's family met Minister for Justice Jim O'Callaghan to press for a public inquiry, after a 2023 scoping inquiry recommended against one and a 2018 Dáil motion calling for a Commission of Investigation has not been honoured.
