  Christopher Miley, 31, was fatally injured at a quarry in Rathcoole, Co Dublin, at approximately 6.45am on 28 October 2021. He received treatment at the scene but later died. L Behan Aggregates and Recycling Ltd and Shannon Valley Plant Hire Ltd were charged under Section 77 of the Safety, Health and Welfare at Work Act 2005 with failing to ensure adequate lighting. The case came before Judge Anthony Halpin at Dublin District Court in May 2023, where the Director of Public Prosecutions directed trial on indictment and the matter was adjourned to July for a book of evidence to be prepared.
