latest first- Inquest finds Kilkenny stolen car crash death avoidable Theft of motor vehicle; fatal collision · Co. Kilkenny
A car was stolen from Green Street, Kilkenny, at approximately 3.30pm on 18 November 2022, with 16-year-old Aoife Cahill sitting in the back seat while her father Pat Cahill was in his office. The driver, 37-year-old Dale Fogarty from Castlecomer, Co Kilkenny, drove at speed before colliding head-on with another vehicle on the N77 between Kilkenny and Ballyragget at approximately 3.55pm. Fogarty was pronounced dead at St Luke's General Hospital, Cahill suffered life-changing injuries, and three occupants of the other vehicle, including Frankie Brennan and Graham O'Flaherty, were taken to hospital with non-life-threatening injuries. Gardaí investigated the theft and collision, and an inquest at Kilkenny courthouse in June 2024 found that toxicology showed Fogarty had multiple drugs and a very high level of alcohol in his system, with the coroner ruling his death was very avoidable.
