Dualtagh Donnelly, a 25-year-old man, cut an artery in his arm by punching a glass panel in a door at his home in Dundalk, Co Louth, in the early hours of October 26, 2015, while intoxicated. An advanced paramedic arrived at 3.29am and the ambulance at 3.45am after taking a wrong exit off the M1 motorway, and he went into respiratory arrest at about 3.41am. An open verdict was returned at the inquest into his death. His partner Lyndsey Cooney sued the HSE in the High Court, including a nervous shock claim for psychiatric injuries. The family settled the action for €125,000 without an admission of liability, a settlement approved by Mr Justice Garrett Simons in December 2021.
