A single-vehicle crash on the M6 at Rochfortbridge, Co Westmeath, on 8 February 2024 was triggered by a defect in the right rear tyre, with the car travelling at 173 km/h. Babak Meshkat, a 46-year-old doctor from Co Galway, was driving his six-year-old daughter Hana Meshkat, who was severely injured and died at Children's Health Ireland at Temple Street nine days later. Meshkat was charged in 2025 and sent forward for trial to Mullingar Circuit Criminal Court, where in June 2026 the original charge of dangerous driving causing death was amended to careless driving causing death and he pleaded guilty. On 24 July 2026, Judge Cephas Power sentenced him to a €5,000 fine and a one-year driving disqualification, holding that a custodial sentence was not warranted.
